A baby has been shot and killed by a toddler who got hold of a loaded gun while in a parked car outside a medical center in Texas, according to authorities.

A 3 y/o found a rifle, in the trunk and fired it?

There's a lot about that that doesn't add up.
Another report says the rifle was in the back of the car.

My SA stated that on October 21st, a San Antonio woman was in her parked car with three children in the backseat, ages 3, 2, and 10 months. There was also a loaded gun located in the back of the car. When one of the older children got ahold of the weapon, they accidentally pulled the trigger, firing a bullet straight into the youngest child.

Why? This happens in the U.S. all the time.
Yep.

It happened to the family of a member of my command. While we were deployed, his eight year old son found his shotgun and shot his six year old daughter by accident. Blew half her torso away.

When my son was at his best friend's house when he was little boy, I forget how old, he found a shotgun behind the bedroom door of his best friend's parents.

Thanks to the many lectures about gun safety I had given him, he didn't touch it and told me about it. I then notified the kid's dad, who was a police officer of all things, and he properly stowed it away.

Apparently the guy had received death threats from a biker gang he had busted, and wanted the gun close at hand.

But still, that was stupid. His son is on the spectrum.

A 3 y/o dug a rifle out of the trunk and fired it? That sounds kind of the unlikely.
Another article says it was in the back of the car where the kids were sitting.

It must have had the safety off. And if single-action, it must have been cocked.

It is hunting season. Where I live. Guns and knives in my truck this time of year. In the cab they are unloaded and the ammo is in the bed of the truck. I go to see my grandbabies this weekend for the first time in a long while. Friday afternoon I will go over that truck with a fine tooth comb and remove any dangerous or sharp items. I am all for the safe ownership of fire arms. I highly doubt the parents did not care, although yes there are those types of people but they are not common. Many people especially urban people were never trained in gun safety. Gun safety courses should likely be required for ownership. As I live alone I do not own gun safes and such as I did when my children were around. My ex wife's new husband is or was the proud owner of my gun safes. At least the such and such couple did not get any of my guns. Lol. A tragedy for sure. The incident is a big punishment but not enough. Some jail time needs to be handed out.
