Why are two people inefficient?
All other agencies function without two. Sharing a redundant command level in one organization is usually not a good idea.

In addition to millions of active duty and reserve federal employees, I would not be surprised that there are even more contract personnel working for the federal government even though the official numbers say something over 500,000. I personally know two federal employees that have been 'working from home' since the COVID lock down and who admit they spend most of their day playing video games and surfing the web or otherwise doing whatever they please. They make sure they answer the phone--mobile phone--if a supervisor calls but they aren't doing a whole lot of work.

Musk and Ramaswamy are tasked with looking at this kind of situation and making recommendations that people either actually work for their very attractive salaries or find something else to do.

Also the government will spend roughly $7 trillion this year alone and I would guess not more than a very few people can name a single thing accomplished with all that money that is of any lasting value to anybody or that has improved the lives of any Americans. But it has exacerbated inflation and hurt a lot of people. I paid $2.27 for a head of lettuce this week which is the most it has ever cost ever. It was 65 cents a head in 2019 and 2020.

Yes. One person in command, other person assisting. Founding fathers, nor military ever envisioned two in command on the same level, and for a very good reason. It is problematic from the start or almost immediately after, and could be downright dangerous in a military or other critical situation.
 
Two heads are better than one. Besides, there is more inefficiency out there than one man can handle.
One assume the department will have a staff. Who they report to, or have to listen to, is a different matter.

I figure they'll work it out just fine. Unless you have ever been part of an effective leaderless team, it is hard to imagine it being effective. It certainly can't work in every circumstance, but with effective communication, in some cases it can be amazing.

Oh, and of course we all know how good those two are at working for other people. I am kind of surprised you haven't suggested two Presidents at once. Nothing quite as effective as rule by committee, eh?
