The door closed by a teacher failed to lock as it was supposed to. Security camera shows teacher closing the door after she saw him crash outside and people yelling he has a gun, then the shooter opened soon after.

In most of the schools where I taught, the doors were set to automatically lock from the hallway when closed. someone wants in, they have to be let in!
