It is not possible with 3rd party payers, private or nationalized. It is only possible, if price negotiations happen between service providers and patients. Has not happened since the 50's, republican or not.

Same deal as with the insurance. By the time you add up all your premium payments, you have already paid for your wildest treatment, whether you had it or not.
