Why has there been no Impeachment Inquiry vote as required?

This is the start of an investigation like any investigation that police do. They do not vote on it.

Committees are formed to investigate allegations. At this point that is all they are just allegations. Certain individuals determine if it should proceed to the next stage

once they put it all together then they send there conclusions to the Judiciary Committee (similar to a grand jury )

They will determine if it should be put to a vote by the house or they can may say that the case is inconclusive and will not be voted on. Which can end it without a vote

That's how it works

The word inquiry says it all at this point in time.

House rules.

While all committees can investigate the smell of every Trump fart, a single committee is designated by the house (Judiciary, presumably), to conduct impeachment investigations.

If you had watched the Lewandowski hearing you would have heard Republicans repeatedly pointing out that it was not a real impeachment hearing, and asking why the House Democrats refused to have a vote to set up real impeachment hearings.
