They feel like something is wrong and they don't know what it is, so the poor creatures fixate on gender but that doesn't help. Nothing helps. They live in an agony because they cannot identify this feeling of otherness that they have. This is the same kind of mental illness that people have that suffer from body integrity disorder. They feel like they should not have arms, legs, they should be blind or deaf, but when they get their desires filled, they still don't feel "right".
