It’s a fairly precise technical legal definition that entails variety factors. If one (as an example) is unable to distinguish the wrongfulness of his conduct, it might very well be that he lacks the required culpable mental state needed by law to commit the crime.

Under those circumstances, the verdict of “not guilty by reason of mental defect” doesn’t exonerate the accused. It just requires that he be put into a secure mental hospital.
