What was the rationale for changing things so that instead of not being able to buy health insurance across state lines, now you can't buy it across county lines?
 
In the Obamacare exchanges insurance availability and cost are based on which county you live in.

This is more restrictive than the restrictions which already existed and which some had been lobbying to remove. It's leading to fewer options and higher premium costs in rural areas than in more populous counties. (to name one consequence of the new restrictions)

You are misreading something somewhere, obviously.

A healthcare provider may refuse to accept all but one particular insurance plan, and that insurance company may only offer that particular plan to a small geographic area.

It would not be ObamaCare saying you can't buy across county lines, it would be both the healthcare provider and the insurance company doing that.
