Here is text from the law:
A covered entity may use or disclose protected health information without the written authorization of the individual, as described in § 164.508, or the opportunity for the individual to agree or object as described in § 164.510, in the situations covered by this section...
(1) Permitted uses and disclosures. A covered entity may use or disclose protected health information for the public health activities and purposes described in this paragraph to:
(i) A public health authority that is authorized by law to collect or receive such information for the purpose of preventing or controlling disease

It seems pretty simple and clear and not really surprising that the government entitles itself to our protected health information as they deem appropriate. Also, I bet lots of vaccine administrators voluntarily share this info with the gov.

It's kind of scary, if you think about how protected information could be misused. The vaccine consent form asks for all kinds of protected information about the individual's health history. Here's the Walgreen's form:
