Your second point is not necessarily true. They can be fearful that the federal government will investigate them if they are Hispanic and a citizen. We are seeing how the government will seize on anything to strip people of their citizenship. There are also programs that can be accessed by legal non-citizens as well as citizens.

Type of residence
Phone number
Name, sex, age, date of birth, race/ethnicity of each person.

Yes, they do. There were two exceptions for representation in Article I: slaves were counted are 3/5ths and Indians not taxed (living on reservations).
The 14th amendment eliminated the 3/5ths clause and the Indian Citizenship Act of 1926 was the end of Indians not taxed. Everyone else is to be counted.
