I do not know the exact reason but generally

Police request an arrest warrant to the DA office. The DA office will approve or deny it based on the evidence presented to them. It is not a judgement of guilt or innocent just the procedures for the arrest when the police have a reason for wanting to arrest the person and they present enough

So yeah mistake can be made. Then the victim can sue the police or state.
