My father is a Ph.D. He went by Dr. his whole working life. Ph.D.s were actually the real doctors back in the day. Calling an M.D. "doctor" was sort of honorary, since a medical school was considered more like a trade school, as opposed to the rigorous scientific standards and the requirement to write a thesis faced by Ph.D. candidates.

My S-I-L is a doctor. Interestingly, specialists are called Mr, Ms or Mrs. ie, if you a cardiac specialist, the peers and nurses call you Mr Smith, not Dr Smith, True story.

I live in Australia. In fact, the first time I made an appointment to see a specialist the PA answered the phone "Mr Smith's offices"...and I said "I'm after Dr Smith" and she laughed and went on to explain to me that specialists are called Mr. I thought it was BS, but I have seen two or three specialists since then and they are all addressed as Mr. I asked my SIL about it (she's a Dr in NZ) and she said, yep, a Mr/Ms has a higher standing in the medical community because they are a specialist.
