It’s not the ambulance, it’s the emergency care you receive once you get to the hospital.
If you want to make ER services a part of police and fire, I have no problem with that. I never have. Of course, those services would be for actual emergencies, not for a cold or other abuses.

But, that is only one small portion of health care services, and again, most ER costs are covered by casualty insurance, which goes through an entirely different process where hospitals end up reducing the costs to get paid (personal injury law suits).

They are really big bills actually. So are all surgeries. I assume when you need life saving surgery you will look for the budget option?

It ends up bankrupting doctors who can't compete and forces others to be efficient. They don't get to jack up their fees and let health insurance cover their losses. They don't engage in unnecessary diagnostics and cut to the chase, because they will not get paid for all the bullshit. The $23 for an aspirin shit immediately gets shot to hell.

But, with nobody able to pay for those services, the providers will be required to compete for available business, and the result is that the cost goes down.

Health Care providers are the only industry that is NOT required to compete with others over prices.

The vast majority of health care transactions aren't emergencies. And even emergencies can be prepared for, and such preparation can include price shopping. But no one will bother if they think someone else will be picking up the tab.

You have again identified the problem, and I think you are right. Beggars cannot afford to value their life more than their budget allows. That means the budget surgeon gets their business, and the expensive guy is sitting around, twirling his scalpels , not getting work.
