The efficiency is measured AGAINST the control group. IOW if you take a population of 100,000 people and 1000 of them get ill, a 90% effective rate would mean that only 100 would get ill. The numbers for the unvaccinated are already part of the outcomes or you did not have a study. A proper study requires a control group. A single study would not reveal hard information either, it needs to be replicated to achieve an accurate number as well which is why this summery includes 176 studies. Those numbers are an aggregate of what those studies reveal.

I don't believe you are correct on that one. Delta came about in India where, at the time, not many people were vaccinated. While not impossible, it is highly unlikely. However, vaccines can lead to mutations.
