the ones that couldn't keep their doctor were on preferred networks b4 the ACA - & when the exchanges were set up, some insurance companies took their more accessible physicians off the exchange networks & put them on their own off exchange networks. the mandated protections were still in force, but the health insurers were able to jack up the premiums when they are off the ACA exchange; something that was not expected - but was legal to do. that's why people 'lost' their doctors.

funny though how any insurer can suddenly change their networks, or if your workplace changes plans/companies - you can lose your doctor anyway. that has always been a huge problem.

some shit policies where it was cheap cheap cheap to have but offered no real bennies - were dissolved because they didn't meet the protections the ACA required - so some lost their policies & therefore their doctors. they then got better policies because they had the mandated coverage.
